Protected Health Information (PHI) includes:

• Medical records and documentation you provide
• VA disability ratings and claim information
• Treatment history and healthcare provider information
• Personal identifiers (name, SSN, DOB, address, phone, email)
• Service-related medical details shared during consultations
• Documents or forms you upload to help with your VA claim

Other Information:

• Contact information for scheduling and communication
• Website usage data for improvement purposes
• Payment and billing information for services rendered

We use your PHI for:

• Treatment: Providing personalized VA disability claim consultation and support
• Payment: Processing consultations, services, and IGY6 Fund donations
• Health Care Operations: Quality improvement, staff training, and business operations

We use other information to:

• Communicate with you about appointments and updates
• Improve our services and website functionality
• Process donations to the IGY6 Fund through secure third-party providers like PayPal

All other uses of your PHI require your written authorization.

• Data Deletion: Request deletion of your personal records after services are rendered

Donations to the IGY6 Fund and payments for services are processed through secure, third-party providers like PayPal. We do not store your credit card or banking information.

We collect website usage data for improvement purposes. This information is not linked to your PHI and is used solely for enhancing user experience and website functionality.

We may update this policy periodically to reflect changes in our practices or legal requirements. The latest version will always be available at www.boots2benefits.com/privacy-policy. Material changes will be communicated to active clients.
